Community Outreach Services

We focus our energy and resources toward community development at a most personal level, where each individual can make an impact. Through person-centered planning and integration support we assist adults with disabilities in functioning with only needed support within their own homes, neighborhoods and communities.

Every person deserves—and is expected—to be an active participant in their community, now and in the future. Our job is to support individuals and families to make that happen.
